- Can we please have a copy of the slides?

Yes, the slides from the Technical Assistance Call will be posted to the Center for Minority Health Services website.
You will need to complete that form as part of your application and upload it in the designated section under the Miscellaneous tab. At this stage, you should fill it out using the partners you are currently working with. If you are awarded the grant, you will then use the same form each quarter for the duration of the grant period.

However, points will not be deducted for omitting items you do not need based on your location (such as hotel accommodation).
The items to consider include:
1. Conference registration
2. Mileage to the event
3. Per diem – may not be needed if you already live or work in the area
4. Hotel accommodations – may not be needed if the staff person can commute daily
Yes, a new position can be created using funds from this grant. If you create a new position, you must identify the staff title under the Personnel section of the budget. If none of the listed titles apply, select “Other” and type in the title followed by “TO BE HIRED.”
In the notepad section, explain why the position does not currently exist and why it is needed. Also describe the duties the new staff member will perform as they relate to carrying out the work of the grant. You may also wish to include why the position is currently vacant and when you expect to hire.
Yes. Whether you are applying under Capacity Building or Implementation, you must partner with a local health department that serves the Healthy Resilient Community region. The local health department must also provide a letter of support outlining their role in the collaboration and how they will help achieve the objectives in the grant work plan.
The key requirement is that the collaboration must be connected to the State Health Improvement Plan (SHIP) or the local health department’s Illinois Project for Local Assessment of Needs (IPLAN). If the work your clinic is proposing aligns with priorities identified in the SHIP or IPLAN, then an existing relationship may be sufficient—but you must explicitly describe this connection in your application.
If your current relationship does not relate to SHIP or IPLAN priorities, you will need to demonstrate how the partnership for this initiative aligns with one or both of those plans.
Yes. The program’s grant manager must be notified immediately of any personnel changes or modifications to work plan activities or budgeted items that differ from what was included in the original application. Prompt communication ensures proper grant oversight and compliance with program requirements.
The local health department is not required to receive any of the grant funding. Their role is to serve as a partner in supporting the objectives and activities outlined in the work plan. However, if you choose to allocate funding to the local health department, they would then be considered a sub‑grantee. In that case, they must complete an Excel budget form, as required for all sub‑grantees.
We received funding in FY25 and FY26. We hope that this unique funding opportunity to support the development of healthy, resilient communities in Illinois will continue. However, future availability is dependent on funding appropriations approved by the Illinois General Assembly and the Governor.

However, please be aware that because this is a competitive funding opportunity, some questions cannot be answered. In those instances, you may receive a response indicating that the question cannot be addressed due to the competitive nature of the process.
