Adverse Health Care Events

LENS (Learning from Events to Nurture Safety) is an Illinois Department of Public Health program dedicated to improving patient safety and healthcare quality across Illinois. The program was established to implement the Illinois Adverse Health Care Events Reporting Law of 2005, which requires the Department to collect reports of adverse health care events from hospitals and licensed ambulatory surgical treatment centers in Illinois.

Reporting is not yet active. IDPH is currently preparing for statewide implementation, planned for first quarter of 2027.  Through secure reporting and statewide learning, LENS will help turn individual health care facility experiences into shared knowledge that strengthens care, supports a safer workforce, prevents harm, and improves outcomes for patients, families, and communities.
