Illinois Healthy Resilient Communities Grant - FY 2027

The Illinois Department of Public Health is seeking to award grants for the development and implementation of healthy resilient communities (HRC). Awarded HRCs will receive funding and technical assistance to support partnership development, expansion, and place-based interventions that address social determinants of health in populations that experience health disparities. This initiative will provide funding for awardees for either Capacity Building or Implementation. The Capacity Building component focuses on partnership development, needs assessment and program planning for future implementation, including the development of a logic model. The Implementation component focuses on partnership expansion and program implementation using a logic model and evaluation.  Awarded HRCs must also ensure that their interventions align with the relevant priorities articulated in the Illinois State Health Improvement Plan (SHIP) and/or the Illinois Project for Local Assessment of Needs (IPLANs).
