The LENS reporting platform is not yet active. Adverse health care event reports are not being collected at this time.
The Illinois Department of Public Health (IDPH) LENS program is actively preparing to implement the Illinois Adverse Health Care Events Reporting Law. Statewide reporting is anticipated to begin in the first quarter of 2027. IDPH will provide advance notice, training, and guidance before the official launch. Hospitals and licensed ambulatory surgical treatment centers will be required to report adverse health care events that occur on or after the official launch date of the LENS reporting system. Events that occur before the launch date do not need to be reported.
Who Will be Required to Report?
Under the Illinois Adverse Health Care Event Reporting Law, 410 ILCS 522, certain health care facilities are required to report adverse health care events to the Illinois Department of Public Health:
- Hospitals maintained by the State or any department including state-operated psychiatric hospitals and hospitals operated by the Illinois Department of Human Services (DHS)
- Hospitals maintained by Illinois public universities or colleges that are supported by tax funds
- Hospitals licensed under the Hospital Licensing Act
- Hospitals organized under the University of Illinois Hospital Act
- Ambulatory surgical treatment centers licensed under the Ambulatory Surgery Treatment Center Act
Pilot participation
Before launch, a six-month pilot will occur to allow selected facilities to test the reporting system and provide feedback. The pilot will be limited to a subset of facilities. To express interest in participating, please complete this interest form. Please note: completing this form does not confirm participation. IDPH will follow up with facilities beginning in May 2026.
