OAPP Creating a Health Care Provider Certification Instructions
1. Sign in to the Illinois Cannabis Tracking System (ICTS): https://etk.icts.illinois.gov/etk-icts-prod/login.request.do.
2. It is important to review the directions at the top of each page until you become familiar with the process.
3. Click on the Health Care Provider Certifications link.
4. For new patients, select +New.
5. The new certification page appears. Select OAPP (Opioid Alternative Pilot Program).
6. Health care provider should have the patient’s Social Security number, patient first name, patient last name, and the date of birth available in order to enter the correct information.
7. Enter the Date of In-Person Physical Exam and the length of time the patient has been under health care provider care in years and months. If this is the first visit, enter 0 years and 0 months. The exam date must be within 30 days of the application submission.
8. Check the Illinois Prescription Monitoring Program (https://www.ilpmp.org/) to verify the patient has not been certified for the Opioid Alternative Pilot Program or the Medical Cannabis Patient Program by another health care provider. Select from the dropdown box if the patient has been prescribed an opioid prescription. In the second dropdown box select if the type of pain is chronic or acute. Select one or more medical diagnosis for which an opioid has been prescribed. Note: Once a certification is revoked, the patient’s current Opioid Alternative Pilot Program application will be revoked.
9. The Certification Status should be auto populated with Eligible.
10. Enter the Health Care Provider PIN created during registration.
11. Read the following text and enter your full name.
12. Click Save at the bottom of the page. The certification list appears where the health care provider can gain access to patient certification records. Note: The patient will sign into their application and be able to add the health care provider certification to their application. If the certification is not used for a Patient Registration application, health care provider certifications will be automatically set to expire after 30 days. In that situation, no manual action is required.
