Minor Patient Registration Reapplication Instructions
** Important Notice: Minor Medical Cannabis Patients' cards are valid until they expire, regardless of the patient turning 18 years of age. **
Once the minor medical cannabis patient turns 18 years of age, they have two options:
- If the patient would like an adult medical cannabis card, they can cancel their current card and apply again as an adult. Cancellation requests will have to email IDPH at DPH.MedicalCannabis@Illinois.gov. In the email, include the patient's name, date of birth, caregiver name, caregiver date of birth, and the minor qualifying patient’s and the minor caregiver’s registration card numbers.

MCG.0000000 - Once the Minor Qualifying Patient’s card has expired and the patient is over 18 years of age, the patient (previous minor qualifying patient) can re-apply as an adult in the Illinois Cannabis Tracking System (ICTS). For Instructions, refer to the MCPP Patient Application instructions.
Note: Before starting an MCPP Patient Application, contact your health care provider. Health care provider certifications are only valid for 90 days from the date of the in-person/telemedicine examination.
Parent of a disabled child who is unable to complete the application for themselves. Unlike with the pediatric application process, you cannot apply as a legal guardian for the patient. The application process must be done with all the patient information, and you will have to add yourself on as a caregiver.
Note: When re-applying as an adult patient, the application might auto-populate with the parent/guardian information instead of the patient’s information (double-check). Make sure you are using the actual patient’s name, otherwise the health care provider’s certification will not link.
